The left column ought to be hidden when tdecmshell is running just one module, same as KDE3 and KDE4 do, and TDE 3.5.x did. Mailing list thread http://trinity-devel.pearsoncomputing.net/?0::14468 points to my R14 screenshots on openSUSE of kcmshell(4) fonts, kcmshell(4) xserver, tdecmshell fonts and tdecmshell xserver, where it can be seen that the Trinity windows include a module selection column that serves no purpose. In fact, it compounds the already existing redundancy shown in this attachment - "X-Server" is displayed 3 times, yet not in the titlebar, where if present would enable distinguishing different control module windows on the taskbar.

Note in attachment that the window was purposely shrunken to the minimum possible size it would allow.

I can confirm that in TDE 3.5.13 and earlier list of icons is not displayed, if is opened just one module. Therefore, it could be called a regression in R14.0.0.

I thought this had been fixed, but it's clearly not in the current r14.0.4 tdecmshell fonts on Xenial.

Not fixed in 14.0.3 on openSUSE 42.1.

Of course not. R14.0.3 was frozen at least 2 months before the fix :-)
The fix is part of R14.0.4 as this bug report indicates (see "Blocks 2575" which is the metabug for R14.0.4)
